Non-Clinical Program Requirements
All new CUIMC students must complete the pre-registration process in order to register for classes. The information listed below reflects the pre-registration requirements for non-clinical students.
Pre-Registration Immunization Form for Non-Clinical Students
In addition to providing the documentation outlined below, all non-clinical students must complete and submit the Pre-Registration Immunization Form for Non-Clinical Students form. We only accept CUIMC Student Health on Haven Immunization forms. Incomplete information will result in registration delays.
Pre-Registration Requirements For Non-Clinical Students
Measles/Mumps/Rubella
Choose one of the following and submit documentation:
- Two doses of MMR vaccine (after 1 year of age) OR
- Two doses of measles vaccine, two doses of mumps vaccine, and one dose of rubella vaccine OR
- Positive titers (IgG) showing immunity to measles, mumps, and rubella
Meningococcal Meningitis Response Form
- Complete the Meningococcal Meningitis Response Form after you receive your Columbia UNI using the Student Health Portal.
- The vaccine is optional.

Tuberculosis Screening
- Complete the online TB risk screening on the Student Health Portal.
- If you screen negative for TB risk, there are no additional requirements.
- If you screen positive for TB risk on the health portal and DO NOT have a prior history of positive TB test:
- Submit documentation of an IGRA blood test (QuantiFERON Gold or T-Spot) no more than six months prior to the program start date.
- If you test positive, submit a Chest X-ray Report dated after the date of the positive test.
- If you screen positive for TB risk AND have a prior history of positive TB tests:
- Provide documentation of a positive skin test or a positive IGRA blood test (QuantiFERON Gold or T-Spot).
- Submit a chest x-ray report dated after the date of the positive test.
