Pre-Registration Requirements

Please read through the pre-registration requirements for your respective program (see below) and leave yourself enough time (approximately two months prior to your deadline date) to work with your clinician to complete all of the requirements before your registration deadline.

After you have been assigned a Columbia UNI, you may register for our Student Health Portal with an active email address. If your Columbia email address is not yet active, please use an alternate email address. 

Log into our Student Health Portal using your Columbia UNI.

Once logged in, navigate to “Medical Clearances” and complete the following sections:

• Health History
• Meningitis Decision
• Insurance Verification
• Notice of Privacy Practice forms
• Tuberculosis (TB) Screening

Fill out your information at the top portion of the appropriate pre-registration immunization form and physical exam form (clinical students only) have a medical provider complete the rest of the form. 

• Pre-Registration Immunization Form for Non-Clinical Students
• Pre-Registration Immunization Form for Clinical Students
• Physical Exam Form for Clinical Students 

We only accept CUIMC Student Health on Haven Immunization and Physical Exam forms. Immunization and physical exam forms should be completed in English by a physician, nurse practitioner, or physician assistant who is not related to you and should contain a signature and office stamp. 

• All laboratory and x-ray results should be submitted with the accompanied laboratory or radiology report. Your pre-registration forms will not be accepted without this documentation. 
• All titer results must be submitted with a copy of the laboratory report, including the laboratory reference range.

Completing Pre-Registration Requirements through Student Health on Haven

A clinician from Student Health on Haven can perform pre-registration health requirement services for a fee if you are in New York City. Please note the following:

• We charge an administrative fee of $95 to students completing any pre-registration requirements at Student Health on Haven.
• Additional fees will be charged for each service rendered (immunizations and titers).
• We do not accept any type of insurance for these services, except titers, and full payment is due at the time of service (via cash, check or credit card).  The cost of titers may be billed to insurance.  

Log into our Student Health Portal using your Columbia UNI.

Once logged in, navigate to “Medical Clearances” and select either “Imm Form - Clinical” (clinical students) or “Imm Form - Non-Clinical” (non-clinical students) to upload the appropriate documentation.

Additionally, submit proof of your COVID-19 Vaccination (clincal students) and Influenza Vaccination (during flu season) along with any applicable vaccination dates.

Once all of your documentation is uploaded in our Student Health Portal, we will review your completed pre-registration forms and documentation.

• If you are missing any information, we will send you a secure message via our Student Health portal to indicate the missing items.
• You will not be cleared for registration until all of the requested documentation has been received and reviewed by a Student Health team member.

After your completed pre-registration package has been submitted and approved, you will receive a secure message through our Student Health Portal and your registration hold will be lifted within 24 hours. 

Please allow at least two weeks from the time of submission for us to approve your documentation and lift your registration hold.
